2. Click the QR Code button

You now have two options:

Option 1 — Scan on Mobile (Recommended)

• Open your phone camera

• Scan the QR code

• This opens the patient experience on your mobile device

Option 2 — Open Link

• Click the provided link

• This opens the patient experience in a new browser tab
